Senior Services Case Manager
Flagler County government is accepting resumes and completed County Employment applications to work as a Senior Services Case Manager with the Flagler County Social Services department.
Duties: Assist seniors 60 years and older and/or their caregivers in identifying physical and emotional needs and problems through an established interview and assessment process; develop a plan for services which addresses these needs; arrange and coordinate agreed upon services; and monitor the quality and effectiveness of the services. Service will be primarily, but not limited to, state program guidelines for Community Care for the Elderly (CCE); Alzheimer’s Disease Initiative (ADI); and Home Care for the Elderly (HCE). Provides case management in accordance with Department of Elder Affairs Programs & Services Handbook. The case manager will provide comprehensible file narratives in each individual file to meet the standards of the Department of Elder Affairs. Receives, processes and tracks requests for service as outlined by the Department of Elder Affairs, Aging Disabled Resource Center waitlist procedures. Successfully completes all state required components of the DOEA 701B Comprehensive Assessment, 701A Condensed Assessment, and 701S Screening Assessment as updated July 16, 2013. Arranges for needed services and oversees service delivery from all vendors. Services will dictated by a completed Care Plan, DOEA form #203A. The Care Plan information will be documented for required entry into the state’s Client Information and Registration Tracking System (CIRTS). Complies with time-critical response to high-risk referrals from the Department of Children and Families (DCF), Adult Protective Services, to resolve the crisis and/or stabilize the household. Maintains client confidentiality, including handling client files according to HIPAA requirements and shredding of sensitive client documents as prescribed by program and organizational policy. Assesses, measures and documents client satisfaction as required. Actively participates in advocacy for the client. Participates in Area Agency on Aging monitoring of programs to meet DOEA requirements. Informs supervisor of all areas of concern and incidents not in accordance with normal daily routine and job function in a timely manner. Attends staff meetings, workshops, seminars, conference and trainings as required. Participates trainings presented by the Flagler County Emergency Operations Center relating to disaster and/or hurricane trainings. Participation will include assignment as applicable during an actual event.
Minimum Qualifications: Bachelor’s degree in social work, psychology, sociology, nursing, gerontology or related field. Year-for-year related job experience or any combination of education and related experience may be substituted for a bachelor’s degree. Experience with medical terminology helpful. Must possess a valid State of Florida Driver’s License. Salary $33,900 DOE.
